When An Accident Occurs You Are Required To:

1. Report the accident by calling 1-800-277-7500. If you have questions about the prescription program, call 1-800-264-8125, ext. 157.
2. Make sure a post accident drug test is administered. Use one of the locations listed here. If you have questions or problems obtaining a drug test, call 1-800-833-9412.
3. Use an AlaMed provider listed here. There is one located in the area where the injury occurred.
